2020/2021 End of the Year base kid survey

Staff members will give you opportunities to complete this online survey between Wednesday, May 26th - Friday, June 4th. We would like to know about your experiences at BASE during this school year. Please ask a BASE staff member at any time if you have any questions about the survey. Click on the link below to begin the survey. It will take about 15-30 minutes to complete the survey.
