SUMMER INTEREST FORM

2025 Summer Program Information

  • Open to all children who are at least 7 years old. 

  • All members must fill out this interest form to start the registration process. 

  • BASE will not be providing morning care during the SDFA Summer School Program

  • Session fees must be paid by the Thursday before each new week for a BASE kid to attend. An invoice will be emailed to families. Invoices will then be sent out via email one week prior to the week starting.

  • All registration and billing will be set up through Brightwheel 

  • No refunds will be given for any fees paid to BASE.

  • A limited number of scholarships are available. Families requesting full or partial scholarships can fill out this form.

  • BASE families must have an account in good standing with BASE before summer program registration can be processed and approved.

  • Fees include large group field trips and a BASE t-shirt. 

  • BASE families will be responsible for sending additional snacks and sunscreen. 

  • We have three spots for 6 year olds. These spots will be filled via lottery. To be eligible, families must have an older sibling enrolled in our afterschool program as of 2/14/2025 AND have a summer registration form submitted by March 31st 2025. The 6 year old spots will be chosen at random the first week of April and all families will be notified. 

  • BASE will not be hosting a Junior Staff program in 2025, but we welcome middle school students (students entering 6th, 7th, and 8th grade) to participate as members.

Registration Process

  1. Complete this Summer Interest Form via Brightwheel (below). You will need to create an account if you do not have one already.

  2. BASE staff will reach out with the official application process via Brightwheel and a link to the Survey Monkey to complete the registration process. 

  3. Once all materials have been received and approved, you will receive a confirmation email from BASE staff.  If you encounter any issues while completing the application, please email emily@basefortatkinson.org.
